Step Number 1

Click on the forms button

Step Number 2

Then click on the create form by using wizard

Then click the drop down arrow list of table and queries and click the the table or query that you want to make a form from.

Step Number 4

In Step 4 you need to figure out which fields you want to use in your form out of the selected table or query.

Step Number 5

Step 5. You need to select the fields you want and click on the > button to move the fields you want to the selected fields window.

Step Number 6

Step 6. Next you need to pick the type of from layout that you want your form to do from the list.

Step Number 7

Step 7. Next you need to pick the style of text and background that you want from the list and then click next.

Step Number 8

Step 8. Next you need to name the form and click on the finish button.
